This program is designed to provide qualified Lowndes County residents with assistance in obtaining life sustaining prescription medication, including medication for hypertension, diabetes, heart conditions, and etc. There is a $400 limit per person per year. Assistance is not available for behavioral or pain medication, hormone therapy, vitamin supplements or over-the-counter medications.

To qualify, applicants must meet specific federal poverty guidelines, must not have insurance or medicaid coverage, must not have medicare Part D and must have insufficient funds to pay for eyecare services out of pocket.

Applicants must visit the LAMP offices for an intial interview where they will be asked to fill out an application form and present the following information: (1) state photo identification, (2) social security card, (3) proof of residency within Lowndes County (e.g. utility bill, lease agreement, mortgage bill, and etc.), (4) proof of household income (e.g. check stubs, social security statements, separation notices, and etc.), and (5) an original prescription or a copy of the medication label or a detailed prescription profile from your pharmacy listing the medication strength, quantity, number of refills remaining, if any, and current price.

Client interviews are held Monday-Thursday 8:30 a.m. to 11:30 a.m. and 1:00 p.m. to 2:00 p.m. No appointment is neccessary
